For your first appointment, you will need:
- To bring valid photo ID and Medical Insurance Information/Cards.
A current driver’s license, passport, military ID, etc. will be fine. - To bring Recent Lab work and Recent Imaging Reports
- To fill out our new patient forms.
You can fill these out online or on-site. Please arrive 15 minutes early if you plan on filling out your forms on-site. - To bring your preferred method of payment.
Payment is expected on the day of your appointment, and we accept VISA, Mastercard, Checks, Debit Cards and Cash.We are currently providers for MOST UNITED HEALTHCARE, CIGNA, FLORIDA BLUE, TRICARE AND MEDICARE PLANS. We also accept PIP insurance for those injured in automobile accidents. Please Call If You Have Another Provider As Some Plans Have Great Out Of Network Benefits.
